Melyse Behavioral a& Wellness requires that you keep a credit card on file with us to cover any fees associated with your treatment. We accept all major credit cards.
Payment is due at the time of service. Failure to provide payment will result in rescheduling your appointment and a late cancelation fee.
Please note that there may be occassions where you will be charged a fee several weelks after a session. This delay may occur due to deductibles, changes in copays, or other insurance-related adjustments.While we strive to charge your card on or shortly after each session, these circumstances may result in delayed charges. If you would like to receive a receipt for your charges, please let us know.
We understand that there may be times in which you need to cancel or reschedule your appointment. Please notify your provider directly at least 24 hours in advance to avoid a late cancelation fee.
Any missed appointment fees will be charged to the credit card on file.
